- You must enter demographic information (name, date of birth, legal sex)
- Access your records
- Export and download PDFs of records directly to keep for records or share as you choose.

Request records through your health care provider. Patients can request copies of their immunization records through their health care provider. All immunization service providers in Alaska are required to utilize VacTrAK to document vaccine administration. VacTrAK does NOT provide access to patients, only to health care providers.

If you are a health care provider outside the state of Alaska, please fax or mail us a release of information form for the patient.
